Join us as we further explore innovative ways in which care is being coordinated to benefit consumers by creating a higher quality, better experience in care delivery across Maryland and Pennsylvania:
- Lisa Hogan, RN, BSN will explore the successful and continuing journey at Frederick Memorial Hospital Home Care: how chronic conditions are being managed through telemonitoring.
- You will learn how Image Exchange is Unifying Maryland’s Radiology Studies through the expertise of Dr. Daniel Durand, Chief Innovation Officer at LifeBridge Health and program leadership with CRISP.
- You will hear success stories of Improving Integrated Care as Ginnifer Minor and Carlin Henry Barmada, Provider Engagement Specialists for Children’s National Medical Center share their Direct Messaging experiences with the Children’s Integrated Quality Network.
- You will learn about LIFE (Living Independence for the Elderly) - the first and largest of its kind, continuing care retirement community without walls. An affordable and pioneering alternative to traditional continuing care retirement communities, this care delivery model allows members to stay in their own homes and receive all the home care and related services they may need as they age.
